Accommodation request templates

Three documents you need to establish AI use as a protected ADA accommodation:

Functional Limitations Statement — A template for documenting how ADHD affects your specific functional domains. Fill in your own examples and attach to any accommodation request.
Accommodation Request Letter — Workplace — A formal letter template for requesting AI tool use as a reasonable accommodation from an employer.
Accommodation Request Letter — Educational Institution — A formal letter template for students requesting AI tool use under Section 504 or the ADA.
